CASE MANAGEMENT/SOCIAL WORK
Case Management is defined as a collaborative process that assesses, plans, implements, coordinates, monitors and evaluates the options and services required to meet an individual’s health needs using communication and available resources to promote quality, cost effective outcomes. Some of the Case Management Department’s major functions are discharge planning, utilization review, quality and infection control monitoring as well as addressing any psychosocial and financial issues that may develop during a patient’s hospitalization. Case Management Staff can also assist patients with Advance Directives.
The Case Management Staff consist of Social Work and Nursing professionals with many years of acute care experience. Case Management involvement can be initiated through practitioner orders however, all cases are screened to identify any high risks that may necessitate involvement.
Case Managers will also act as a liaison between the hospital, physician and insurance companies.
