What is Patient Registration?

Central Registration can be reached at 585-395-6095 extension 4292 for any questions regarding the above mentioned.

 
 

Pre admission testing:

Patient may download copy of the Health survey questionnaire that is needed on the day of appointment in Pre admission testing.  Along with a map to show where you need to be on the day of appointment and day of surgery.
 

Radiology:

Patient may download copy of the required questionnaires for the following procedures; MRI, Mammograms, Dexa scans, CT scans and a general medical history.
 

Centralized Scheduling:

Can be reached at 585-395-6048 for inquires regarding scheduling appointments for radiology at this time.
